Fees after May 31st:
1st player $125, 2nd player $110, 3rd player $100, 4th player $50, U5 player $60.
You may be placed on a waiting list, as spots usually fill up by the end of the registration window!

What to mail:
- If you are a returning Millbrae player from last year, update your child’s registration on www.eayso.org and print two copies of the registration along with a check made out to Millbrae AYSO and mail to :

- For registering any new players, please register at www.eayso.org and print two copies of the registration, along with a check made out to Millbrae AYSO and a copy of your child’s birth certificate and send to:

- If you are a returning Millbrae player but did not play in the previous immediate season or if you are a returning player from another region, please re-register at www.eayso.org and send two copies of your registration and a copy of birth certificate or passport and check to the P.O. Box listed above.
Additional notes:
Photos (1x1½ in) required for any players who will be 10 and older as of August 1st (for player ID cards).
